Biopsychosocial Model of Pain. The biopsychosocial model of pain was introduced by Dr. George Engel in 1977. This widely accepted theory suggests that pain is a result not only of biological factors but also of psychological and sociological factors. The biopsychosocial model is now widely accepted as the most heuristic approach to chronic pain. With this model in mind, a review of the basic neuroscience processes of pain (the bio part of biopsychosocial), as well as the psychosocial factors, is presented. This approach to pain, which takes into account a number of different factors, is known as the biopsychosocial model of pain.

Indeed, as reported by Gatchel, 4 Figure 1 presents a conceptual model of these interactive processes involved in health and illness.

This model highlights the dynamic relationships among a variety of biological, psychological, and social factors that can modulate a person’s experience of chronic pain. In their BPS model of chronic pain, biological and psychological factors consist of central (i.e., biological, cognitive, somatic, affective) and peripheral (i.e., autonomic, endocrine, immune) processes.
